Eligibility Criteria:

  • 50+ years of age
    Score > 12 on Australian Diabetes Risk Assesment Tool
  • Do not have any of the following conditions: Pregnancy, psychiatric illness, cancer within 3 months of commencing the course and substance abuse.
  • Automatic entry for patients who have had Gestational Diabetes and Ischeamic Heart disease (Acute Myocardial Infarction, Angina etc)

GP Referral Process:

  • Ensure the patient meets all eligibility criteria:
    a. Ask patient to complete Australian Diabetes Risk Assessment Tool
    b. Order FBG test if not completed within last twelve months (OGTT if result FBA >5.5,<7.0,
    LIPIDS: TC,HDL,LDL,TRIGs).
  • Complete LIFE! Referral Form – Refer to PACE (Note: this form can be down
    loaded from diabeteslife.org.au under Health Professionals> LIFE! Program referral form, also
    on Medical Director.
  • Fax copy of Life Referral Form, blood and lipid profiles, patient contact details and medical
    history to (03) 9770 6776 (all referrals to be faxed to this number). .

Participant Investment:

$50 out of pocket for High Income Earners only.
