Supporting older Australians to stay safe, active and independent at home

Remaining at home for as long as possible is a deeply valued goal for many older Australians. Through the Support at Home program, PACE provides coordinated Exercise Physiology, Occupational Therapy and Dietetics services to help clients maintain mobility, safety, health and confidence in their daily lives.

Whether it’s preventing falls, improving strength and balance, managing changes in health, or making the home environment safer, our team works closely with each client to support meaningful independence. We focus on practical, achievable strategies that enhance quality of life and reduce the need for hospitalisation or residential aged care.

All services are delivered in line with Support at Home program requirements, clinical best practice and strict compliance standards - ensuring safe, high-quality, ethical care. We collaborate with providers, carers and families to ensure support is integrated, coordinated and responsive as needs change.

With a multidisciplinary team and a commitment to dignity, safety and wellbeing, PACE helps older adults stay active, stay connected and stay at home for longer.

Who We Work With

This service is designed for older adults receiving a Support at Home Package who require:

  • Capacity building to maintain or improve independence

  • Falls-prevention support

  • Assistance with daily living tasks

  • Home modifications or assistive technology

  • Nutrition intervention to support health, wellbeing and chronic condition management

Clients access our services through their Support at Home Package Provider, who arranges referrals on their behalf.

Our multidisciplinary team supports clients with a range of age-related needs, including:

  • Falls prevention and dynamic balance training

  • Reduced strength, mobility or endurance affecting daily living

  • Capacity building to maintain independence at home

  • Declining nutritional status, weight loss or chronic disease risk

  • Environmental and functional barriers requiring home modifications or equipment

The goal is to help clients remain safely at home, improve quality of life, and reduce the need for hospitalisation or residential aged care

As part of the Support at Home program, PACE provides Exercise Physiology, Occupational Therapy and Dietetics to support older adults to live safely and independently at home.

Occupational Therapy

Our Occupational Therapists help clients live independently by assessing daily activities, home environments and functional challenges. Support may include home modifications, assistive technology prescriptions, falls-prevention strategies and capacity-building interventions to improve safety, mobility and participation in meaningful daily activities.

Exercise Physiology

Our Exercise Physiologists provide evidence-based exercise programs to improve strength, balance and endurance. Interventions focus on reducing falls risk, restoring functional capacity, and supporting clients to confidently manage daily tasks. We prioritise self-management by developing safe, personalised home exercise programs and training support workers or carers as needed.

Dietitians

Older adults have unique nutritional needs, and Dietitians play a key role in supporting health, wellbeing and quality of life as people age. This includes ensuring meals meet individual preferences, textures and cultural needs, as well as managing health conditions through targeted nutrition therapy.

How to Access This Service

We cannot accept direct referrals.

To access PACE through the Support at Home program:

  1. Speak with your Support at Home Package Provider, and,

  2. Request that they send a referral to PACE for the services you require.

Your package provider coordinates all referrals, funding approvals and communication.
